Empowering Care
comprehensive care coordination
MW Care Associates is a family-led caregiving initiative, documenting firsthand experience in comprehensive care coordination, healthcare advocacy, fiduciary responsibilities, and compassionate support while navigating the complexities of long-term care.
Our Story
MW Care Associates was founded by sister and brother, Samantha Rosano and Travis Wojcik, after firsthand experience with the challenges of caring for their mother as her cognitive needs increased.
While balancing full-time careers and their own personal responsibilities, they did not want to choose between becoming full-time caregivers or placing their mother's care in the hands of others. Instead, they built a system that allowed them to remain actively involved while providing the compassionate, dependable support she deserved.
Samantha's background in case management, behavioral health, client advocacy, comprehensive care coordination, and community resource navigation helped coordinate medical care, services, benefits, and family communication. Travis focused on companionship through conversation, activities, outings, and meaningful social connection. Together, they learned that exceptional care is about more than managing appointments and daily tasks - it's about preserving dignity, independence, and human connection.
Today, MW Care Associates combines lived experience with professional expertise to provide compassionate, non-medical caregiving, care coordination, advocacy, and family support.
Philosophy
Tailored Care
Every individual deserves care that reflects their unique needs, preferences, and goals. It is important to take time to understand the whole person, creating thoughtful, individualized support that promotes dignity, independence, and quality of life.
Advocacy
We believe everyone deserves someone who will listen, ask questions, and speak up when it matters most. Whether coordinating services, communicating with healthcare providers, or navigating complex systems, advocating with integrity and purpose is paramount.
Coordination
Exceptional care requires more than compassion - it requires organization. Thoughtful planning, clear communication, and proactive coordination create better outcomes and peace of mind for those facing complex caregiving challenges.
Meaningful Connection
Care is about more than completing daily tasks. Through companionship, engagement, and genuine human connection, it is integral to enjoy the moments that bring purpose and fulfillment.

Samantha Rosano
Samantha oversees every aspect of care coordination and day-to-day care management, ensuring that medical, personal, and financial responsibilities work together to support each client's well-being. Drawing on her professional background in behavioral health, case management, client advocacy, and community resource navigation, she develops individualized care plans, coordinates with healthcare providers, advocates for client needs, and navigates complex healthcare and community systems.
Serving as both her mother's attorney-in-fact under a durable power of attorney and healthcare proxy, Samantha has extensive experience managing the medical, financial, administrative, and fiduciary responsibilities that accompany long-term caregiving. She coordinates medical appointments, communicates directly with physicians and specialists, attends appointments alongside her mother, monitors changes in cognitive and physical health, facilitates referrals, coordinates medications and treatment plans, and advocates to ensure her mother receives timely, appropriate care.
Her responsibilities also include bookkeeping, financial recordkeeping, budgeting, bill payment, tax organization, insurance and benefits coordination, reimbursement tracking, document management, and exercising fiduciary responsibilities in accordance with her legal authority and always in her mother's best interests.
Samantha believes exceptional care extends beyond meeting physical needs - it requires thoughtful coordination, strong advocacy, financial stewardship, accountability, and a deep commitment to preserving each person's dignity, independence, and quality of life.
